Conneticut

In order to certify patients for medical cannabis in Connecticut, you’ll need to: Confirm that you meet the Physician Requirements and Eligibility. Create a DAS Business Network Account. Log in to the registration system using your DEA number and last four digits of your Social Security Number. Be prepared to obtain contact information for your patient, including valid e-mail address and primary contact number.

Florida

Before being approved as a qualified physician, and before each license renewal, a physician must successfully complete a 2-hour course and subsequent examination offered by the Florida Medical Association or the Florida Osteopathic Medical Association.

Maryland

Provider Registration: Gather required information. (Email address, MD Board of Physicians License Number, Controlled Dangerous Substances Registration Number) Complete and submit online application. Verify your email address.

New Jersey

Physicians who wish to participate in the program must: Hold an active New Jersey Medical license in good standing issued by the NJ Board of Medical Examiners. Possess an active controlled dangerous substances registration issued by the NJ Division of Consumer Affairs that is not subject to limitation. Practice within the State of New Jersey.

New York

Physicians who wish to participate in the program must take part in a four-hour course before registering as a practitioner through the Health Commerce System.

The Department has approved two course-providers:

The Answer Page – The online course will cost $249 to take. Successful completion of the course will provide 4.5 AMA PRA Category 1 CME CreditsTM, 4.5 CPE credits and 4.5 CNE credits. The Medical Cannabis Institute – The online course will cost $229 to take. Successful completion of the course will provide 4 AMA PRA Category 1 Credits™ and 4 ACPE Credits.
